Cancellation Fees
If a non-refundable and non-transferable booking fee was required when booking, it will be credited towards the service and the remaining balance is due on the day of the appointment. The remaining balance will be paid in cash (preferred), Venmo, or card. If you would like to pay through Venmo, please label as “Food” or use EMOJI’s please!
If you booked directly through the site and a credit card was put on file: Credit cards must be put on file to hold your appointment. Failing to reschedule your appointment within the time permitted and/or no-shows will be charged 50% of your total cost as a cancellation fee. If you wish to rebook again, you must book directly through Mai Dao and pay in full upon booking your new appointment. If you are no-show or late to your appointment, we have the right to keep the deposit for the time we have reserved for you. I understand that I will not be entitled to any refunds whatsoever; if I do not like the finished result, or for any other reasons, once the procedure has begun or taken place.
Initial Visits
During the initial visit, you MUST bring your government issued photo ID to confirm if you are of age to recieve a tattoo. A mandatory consultation will determine the client’s desired brow look and whether it is obtainable.
Rescheduling/Cancellation/No Show/Lateness
Plan for your appointment. If you live far, plan for your arrival time. We have a zero-tolerance policy for late cancellations. Late arrival shortens your time of service causing us to rush our work leaving you unsatisfied. It also pushes back the appointments of those scheduled after you. Everyone’s time is precious so please be considerate – of our staff and other customers. If you must reschedule, you are to notify us 72 hours (3 days) before your appointment. If you are 15 minutes late, last minute cancellation, no show, or rescheduling under 72 hours (3 days), you will forfeit your session and be charged the cancellation fee. You may only reschedule ONCE. When you do a last-minute cancellation, we lose the opportunity to provide services to other potential clients. We respect your time, please respect ours. Due to limited appointment availability, we may not be able to reschedule your appointment until a much later date. Our goal and mission are to satisfy clients who need our services first. Should you need to reschedule or make changes to your appointment; phone calls, voicemails, texts, Facebook messages, and emails must be received at least 72 hours before your scheduled appointment.
No Children/Pet
In our most modest way, we have a no children/pet policy, for the safety of your child/pet, due to cross contamination, interruption, reachable chemical or sharp disposals, and other clients who are currently having treatments done, we ask that you do not bring your child to your appointment. Appointments will be forfeited if you bring your child. We thank you for putting your child/pet’s safety first, as well as respecting our policies. DUE TO COVID, PLEASE COME ALONE TO YOUR APPOINTMENT, UNLESS YOU NEED A TRANSLATOR. IF YOU SHOW UP WITH A PERSON, THEY WILL HAVE TO STAY IN YOUR CAR.
Brow Perfecting Sessions (FOR EXISTING CLIENTS ONLY)
1-3 Months Follow Up Touch-Up must be scheduled within 1-3 months from your last session.
4-12 Months Touch Up must be scheduled within 4-12 months from your last session.
1-2 Year Touch Up must be scheduled within 13-24 months from your last session.
2-3 Year Touch Up must be scheduled within 25-36 months from your last session.
3-4 Year Touch Up must be scheduled within 37-48 months from your last session.
Anything beyond 4 years will be charged as a new set.
Price varies and must be scheduled within the permitted timeframe. To keep your brows looking their best, it is recommended that you have a color refresh every 18-24 months.
We only offer perfecting sessions on brows that were done by Brows by MDV. PRICES ARE SUBJECT TO CHANGE WITHOUT NOTICE. We will always honor the pricing that was applicable at the time the client made the booking fee.
No refunds
All service sales and gift cards are nonrefundable.
PHOTOGRAPHY & MEDIA RELEASE
I grant to Brows by MDV, its representatives and employees the right to take photographs of me and my property in connection with the above-identified service. I authorize Brows by MDV, its assigns and transferee’s to copyright, use and publish the same in print an/or electronically.
I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be I agree that Brows by MDV may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and web content. These pictures may be used in all aspects of social media including but not limited to Facebook, Instagram, TikTok, and websites.
My consent is freely given as a public service to Brows by MDV, without expecting payment. I release Brows by MDV and their respective employees, officers and agents from any and all liability to which may arise from the use of such news media stories, promotional materials, written articles, videotape and/or photographs.
I have read this release before signing below, and fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that failure to do so will be interpreted as a free and knowledgeable acceptance of this release.
BEFORE YOUR APPOINTMENT (brows only)
If you are on any medication prescribed from your physician, see your doctor for advice before you stop taking it.
To ensure the best results, make sure your health is in good condition. Get enough rest, eat well, and exercise.
Do not wax/pluck, I will do it for you. I will need to see your natural hair growth.
PLEASE DO NOT TINT YOUR BROWS!
One week before: stop taking the following supplements, Vitamin E, B6, Omega 3, Gingko Biloba, and St. John’s Wort, as they contribute to thinning the blood and may affect anesthetic efficacy. It may cause the healing process to be slow and poor. You may resume taking your vitamins 72 hours after the appointment.
72 hours before and after your appointment: avoid alcohol and medications such as aspirin/ibuprofen as it may cause more inflammation and slow down the healing process.
At least 4 weeks before: no Botox, laser, chemical peels, microneedling, or SUNBURN.
If you have formed any skin irritation or blemishes around the eyebrows, please reschedule ASAP.
ON THE DAY OF YOUR APPOINTMENT (brows only)
Please shower before you come to your appointment, it’s recommended that you refrain from getting the treatment area wet for a couple of days. Bathing is recommended for a few days after the procedure. (Post care will be instructed with given care instructions to take home)
You can wear your brows like how you usually would like it to be drawn, that way I can understand how you would like your brows.
Do not apply any lotion on the treatment area.
Do not wear eye contacts, please bring your glasses.
Avoid caffeine.
PRECARE (eyeliner only)
Remove lash extensions at least (2) weeks prior.
No lash lifts and/or tinting at least (2) weeks prior.
Discontinue use of lash growth serums/oils at least (4) weeks prior. *Causes eyes to be raw & sensitive, and creates more lymph fluids and retention can turn grey.
No Botox or fillers to eye area at least (4) weeks prior.
Discontinue retinol, vitamin A and C at least (6) months prior.
Sleep and rest well.
Arrive with no contact lenses.
Arrive with no eye make up (ideally, no make up at all).
No eye creams on day of.
FINE LINE TATTOOS
Finger/hands/feet placements generally heal unpredictably due to the skin thinness. They are super cute, but don’t retain well or the ink blows out. These areas sheds at least 10x’s faster than the rest of our body so you will need to touch them up every so often. Even with that being said, people still like to get it done, therefore I charge more for those areas & touch ups will be charged an extra fee. I do highly suggest the placement to be elsewhere. If any touch-up work is needed, I agree that the work will be done at my own expense.

REMINDERS
If you are experiencing any SYMPTOMS of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell, we ask that you stay home for 14 days until you feel better. If you have an appointment and you start to develop these symptoms, we ask that you immediately contact us to reschedule or we will send you home.
If you arrive early, please text us that you are outside. We will call you once we are ready for you to come in. Masks are not required anymore, but you may bring one if you’d like.
If you have not filled out your online form at home (link will be sent to you the day before your appointment), please remember to bring your govt. ID on your initial appointment.
